Notes on Condition:  The item is in poor condition, with various wear.  The cover was split down the spine and has been taped back together.  The pages of the booklet are not attached to the cover, and the outer sheet (pages 1-2 & 63-64) is missing.  The two sheets at the centre of the booklet (pages 29-36) are loose from the staples, apparently in relation to significant staple rust.
